High-level endoscope disinfection processes in emerging economies: financial impact of manual process versus automated endoscope reprocessing

背景
軟性内視鏡の使用は世界的に急増している。資源が限られた国で広く使用されている高水準消毒には、完全に手動の洗浄・消毒のほか、自動化内視鏡再処理機を用いる方法などがある。不適切な再処理はどの段階であっても汚染をもたらす可能性があり、その結果、患者や医療制度に影響を及ぼし得る。
目的
インド、中国、およびロシアにおいて、ガイドラインで推奨されている自動化内視鏡再処理機を導入することによってもたらされると考えられる成果を、生産性、内視鏡修理の必要性、および感染伝播リスクという 3 つの特性で手動消毒と比較すること。
方法
ピアレビューを経た公表文献および国別の市場調査データを用いた経済モデリング。
結果
生産性の向上により収益の増加が望める国々では、自動化再処理への転換により財務業績に対する直接的なプラスの影響が認められ、中国では 14 か月以内、ロシアでは 7 か月以内で投下資本が回収された。インドでは、自動化再処理により得られた軽減コストと収益は、自動化再処理機の運転コストとほぼ同額であった。
結論
インドおよび中国の内視鏡使用施設では、調査報告にみられる手動洗浄による現行の内視鏡再処理手順によって患者が病原体に曝露するリスクがあり、その結果、感染が生じる可能性がある。ロシアと中国の内視鏡使用部門では、世界消化器病学会（World Gastroenterology Organization）が推奨する手動洗浄から自動化再処理機への転換によって、コストと収益が見合い、直接的な財務利益が得られる可能性がある。

監訳者コメント：
自動機器による消毒と用手的な操作による消毒での内視鏡再処理を、生産性、内視鏡修理の必要性、および感染伝播リスクという 3 つの特性で比較した論文である。B 型肝炎の有病率が高い国では、不十分な洗浄・消毒は高リスクである。しかし、実際の洗浄にかける時間は国によっては不十分な可能性が示唆された。高水準消毒が必要とされる領域では、感染の危険回避が最優先であろう。
