The economics and timing of preoperative antibiotics for orthopaedic procedures

背景
抗菌薬による手術部位感染（SSI）予防効果は、手術開始前の投与時期に依存する。しかし、現行の投与時期と推奨には大幅な相違がある。
目的
整形外科手術時の術前抗菌薬投与の病院の視点からみた経済的価値が、その投与時期によってどのように異なるかを明らかにすること。
方法
病院の視点からの術前抗菌薬投与の電子的意思決定モデルを作成し、オペレーション解析を実施した。ベースライン解析により現行の投与時期を明らかにするとともに、追加解析により様々な投与時期、推奨ガイドライン遵守率、投与から手術開始までの時間間隔目標値を用いたシミュレーションを行った。
結果
手術前 0 ～ 30 分の抗菌薬投与によりコストと SSI が最低となった。オペレーション解析により、術前の操作に基づく投与時期の評価では、最適な抗菌薬投与時期は麻酔の準備後から切開までの間であり、この間の時間の 92.1％は、抗菌薬投与の最適な時間間隔（切開前 0 ～ 30 分）の範囲内であった。この術前の操作に基づく投与時期の遵守率が 80％から 90％に改善すると、1 年間で 100 件の整形外科手術を実施する病院 1 施設あたり、年間 447 ドルのコスト削減がもたらされることとなった。
結論
本研究は、抗菌薬投与時期の改善による想定コスト削減額を定量的に評価したものであり、今後は病院がこの問題に対処するために支出すべき資金の指針となると考えられる。
