心臓の大手術後のルーチンの声門下分泌物吸引：人工呼吸器関連肺炎の発生率への影響

48 時間以上の人工呼吸を必要とする患者に対しては、声門下分泌物吸引が推奨される。心臓の大手術後の全患者に対するルーチンの声門下分泌物吸引の導入の影響を評価するため、声門下分泌物吸引実施前後の人工呼吸器関連肺炎（VAP）発生率、人工呼吸器使用日数、および抗菌薬コストを比較した。介入前と介入後の結果は、いずれも 1,000 日あたりで VAP 発生率 23.92 対 16.46（P ＝ 0.04）、抗菌薬コスト 71,384 ユーロ対 63,446 ユーロ（P ＝ 0.002）、および人工呼吸器使用日数 507.5 日対 377.5 日（P ＝ 0.009）であった。心臓の大手術を施行した全患者に対して、麻酔導入時点からルーチンの声門下分泌物吸引を実施すべきである。
