Extended-spectrum beta-lactamase-producing Enterobacteriaceae carriage upon hospital admission: prevalence and risk factors

本研究の目的は、入院時の基質特異性拡張型β-ラクタマーゼ（ESBL）産生腸内細菌科細菌保菌者の特定における監視培養の意義について評価すること、および保菌のリスク因子を特定することである。1 週間にわたる全入院を、この前向き横断研究の対象とした。
　スクリーニングを実施した患者 525 例中 56 例が ESBL 産生腸内細菌科細菌陽性であり、その半数はスクリーニングのみによって特定された。独立リスク因子として特定されたのは、ナーシングホーム入居、前年の入院、抗菌薬治療歴、および ESBL 産生腸内細菌科細菌保菌歴の 4 つであった。スクリーニングを実施した患者の 50％以上が 1 つ以上のリスク因子を有していた。1 つ以上のリスク因子を有する標的集団をスクリーニングすることにより、全陽性患者の中の 87.5％が特定された。

監訳者コメント：
介護施設と医療機関のいずれにおいても ESBL が拡散しており、先進国では問題となっている。どちらか一方の感染予防では、すでにままならない状況にまで至っている。医療機関は介護施設や老人保健施設等の感染予防の指導を行うことが必要である。
