腎臓病患者の血流感染症:転帰不良および死亡のリスク因子

背景
腎臓病患者の血流感染症(BSI)に関する情報はわずかであり、その多くは特定の患者群から得られたものである。
目的
腎移植レシピエントや慢性腎不全患者(透析あり、またはなし)を含む、腎臓病患者の非選択集団における BSI の特性を評価した。
方法
大規模教育病院の腎臓病科を受診した全 BSI 患者を対象とした後向きコホート研究。事前に規定したプロトコールに従ってカルテレビューを行った。標準的な定義を使用した。
結果
全体で 155 件の BSI エピソードが 108 例に認められ、発生率は 1,000 入院あたり 77.3 件、100 患者年あたり 4.5 件であった。BSI 発生率が最も高かったのは血液透析患者であった。微生物の内訳は、グラム陰性菌 52.3%、グラム陽性菌 46.5%、および真菌 1.2%であった。最も高頻度に分離された微生物は大腸菌(Escherichia coli)(27%)であった。BSI の分類は、原因不明の菌血症(29.7%)、尿路感染症(23.2%)、血管アクセス感染症(17.4%)、およびその他(29.7%)であった。敗血症性ショックまたは多臓器不全が 18 例(11.6%)に発生し、持続性菌血症も同等の割合で発生した。粗死亡率は 14.6%であった。死亡のリスク因子は、Charlson 併存疾患指数高値、持続性菌血症、および発熱がないことであった。
結論
腎臓病患者、特に血液透析患者の BSI 発生率は高い。BSI エピソードの原因微生物として優勢なものはグラム陰性桿菌であった。腎臓病患者には BSI 関連の疾患および死亡が多くみられた。死亡のリスク因子は、Charlson 併存疾患指数高値および持続性 BSI であった。BSI エピソード中の発熱は保護的な因子であることが示された。
