Measles immunity and measles vaccine acceptance among healthcare workers in Paris, France

背景
フランスなどの欧州では 2008 年から麻疹のアウトブレイクが持続している。免疫がない医療従事者が患者に接触し、感染を拡大させる可能性がある。
目的
本研究の目的は、当院の医療従事者の麻疹に対する免疫状況およびワクチン接種意図を評価することである。
方法
2011 年 4 月から 6 月にパリの大学病院 3 施設で医療従事者 351 名を対象として調査研究を実施した。登録時に下記のデータを収集した。年齢、所属部門、職種、麻疹感染歴およびワクチン接種歴、麻疹の血清検査施行歴、および血清陰性の場合の麻疹ワクチン接種意図。ELISA による麻疹検査キット CAPTIA^® を用いて、血清中の特異的抗麻疹 IgG 抗体の有無を調べた。
結果
参加した医療従事者の平均年齢は 36 歳（範囲 18 ～ 67 歳）、278 名（79.2％）が女性であった。全体で 104 名（29.6％）が麻疹感染歴があると申告し、90 名（25.6％）が麻疹ワクチン接種歴がないと申告した。本研究の対象者 351 名中 322 名（91.7％）は麻疹に対する免疫を有していた（IgG ＞ 90 mIU/mL）。免疫がないことのリスク因子は、年齢（18 ～ 29 歳、30 歳以上との比較による補正オッズ比 2.7、95％信頼区間 1.1 ～ 6.9）、麻疹感染歴またはワクチン接種歴がないことであった。対象者が自身の結果を知る前のワクチン接種意図は全体で 78.6％であった。
結論
本研究の医療従事者コホートの 8.3％が麻疹に対する感受性を有しており、その多くは 30 歳未満であった。麻疹ワクチンの接種意図は高かった。医療環境でのワクチン接種キャンペーンは、特に医学生と若年の医療従事者を対象とすべきである。
