Evaluation of a storage cabinet for heat-sensitive endoscopes in a clinical setting

背景
ほとんどの国では、内視鏡は前回使用後に洗浄・消毒済みであっても、毎回の検査開始前に消毒または十分な洗浄を実施することとされている。非耐熱性内視鏡用保管キャビネット（SCHE）はいくつかのものが市販されており、SCHE 製造業者が検証した規定の期間は、洗浄済み内視鏡の微生物学的品質が維持されるように設計されている。SCHE を使用することによって、内視鏡の再消毒が必要とされるまでの保管期間が延長する。
目的
SCHE（DSC8000、Soluscope SAS 社、Marseilles、France）の有効性を臨床環境で評価すること。
方法
SCHE で 72 時間保管後の内視鏡（第 I 群）の微生物学的品質の評価を行い、清潔で乾燥した専用キャビネットで 72 時間保管し、朝の消毒を実施していない内視鏡（第 II 群）および清潔で乾燥した専用キャビネットで 72 時間保管し、朝の消毒を実施した内視鏡（第 III 群）の微生物学的品質と比較した。各群とも 41 本の内視鏡をサンプルとして、微生物学的品質の評価を行った。National Technical Committee on Nosocomial Infection が 2007 年に発表したガイドラインに従って、内視鏡の汚染レベルを分析した。
結果・結論
SCHE は、医療従事者が十分な研修を受け、すべての実践が検証された質保証プロセスの枠内で行われる場合は、内視鏡の微生物学的品質の維持に寄与する。

非耐熱性内視鏡用保管キャビネット（storage cabinet for heat-sensitive endoscopest；SCHE）とは、専用の保管コンテナとラックの組み合わせによるシステムであり、欧米で販売されている。日本では見かけないが、マネジメントがきちんとなされていれば特段問題はないというのが本論文の主旨である。
