Investigation of nasal meticillin-resistant Staphylococcus aureus carriage in a haemodialysis clinic in Japan

日本の血液透析施設の患者および医療従事者を対象として、メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）鼻腔内保菌の調査を実施した。MRSA 保菌は調査 1 年目は 112 例中 10 例（8.9％）、2 年目は 103 例中 4 例（3.9％）に認められた。MRSA の全分離株がブドウ球菌カセット染色体（SCC）mec II 型また III 型を保有しており、医療関連株として一般的な clonal complex 5 に分類された。パルスフィールド・ゲル電気泳動法により、水平伝播は同一セッションの 2 組の患者のみに生じたことが示唆された。医療従事者 54 名中 1 名が、患者株と遺伝学的に関連のない MRSA を保菌していた。米国疾病対策センター（CDC）の勧告に基づく当施設の感染制御対策のため、血液透析室での MRSA の拡散はわずかであったと考えられた。
