Hand hygiene after touching a patient’s surroundings: the opportunities most commonly missed

背景
医療従事者は、感染伝播における環境表面の役割を一般的に過小評価しており、環境に接触後の手指衛生の遵守率は、患者への直接の接触後と比較して一般的に低い。それ以上に伝播が進むリスクを低下させるために、医療従事者は手指洗浄を行う必要がある特定の作業やイベントを認識しておく必要がある。
目的
救急部門および一般診療部門の医療従事者の移動状況を観察し、最も頻度が高い移動経路、および最も接触頻度が高い表面との接触時の適切な手指衛生の有無を明らかにすること。
方法
多床室および隔離室で unobtrusive observation^※による 90 分のセッションを 58 回実施した。リンク解析により、ある場所から別の場所への医療従事者の移動と、動作の頻度を記録した。世界保健機関（WHO）の「手指衛生の 5 つの機会」の観察ツールを用いて手指衛生の監査を行った。
結果
救急部門では、大部分の移動は病床スペース内で行われていた。最も接触頻度が高い表面はベッドサイドのコンピューターと器具トロリーであり、患者との接触直後に多かった。一般診療部門では、病床スペース間での移動がより多く、観察された手指衛生実施率は 25％から 33％の範囲であった。診療部門の種類にかかわらず、医療従事者が隔離室に入室した直後に患者と接触した際に観察された手指衛生の遵守率は 30％未満であった。
結論
低リスクであると考えられる作業時であっても細菌の伝播が起こり得ることを、医療従事者は認識している必要がある。教育・介入プログラムでは、病室のコンピューター、カルテ、およびドアハンドルが汚染されている可能性に注意を払うべきである。
