Prevalence of resistance to antiseptics and mupirocin among invasive coagulase-negative staphylococci from very preterm neonates in NICU: the creeping threat?

新生児集中治療室（NICU）では、感染制御のための局所製剤の使用が増加している。本研究では、カテーテル関連血流感染症を有する極早産児から検出されたコアグラーゼ陰性ブドウ球菌（CNS）分離株 51 株の調査を行った。このうち 41.2％は、少なくとも 1 種類の生体消毒薬に対する感受性低下を示しており（クロルヘキシジン 12％、ベンザルコニウム 24％、アクリフラビン 33％）、61％はムピロシン耐性であった。PCR 法による CNS からの qacA/B 遺伝子、mupA 遺伝子、または両遺伝子の検出率は、それぞれ 59％、63％、および 49％であった。表皮ブドウ球菌（Staphylococcus epidermidis）の 76％（パルスフィールド・ゲル電気泳動で特定された 5 つのサブグループすべて）とスタフィロコッカス・カピティス（Staphylococcus capitis）の 11％（3 つのサブグループ中の 1 つ）は多剤耐性であった。低濃度の液体消毒薬による皮膚消毒、およびムピロシンの適応外使用を効果的に行うためには、これらの薬剤の管理プログラムが必要である。

監訳者コメント：
CNS の消毒薬への感受性の低下は気になるところであるが、実用濃度における感受性への影響には言及されていない。これまでの研究では臨床的には問題がないと考えられているが、使用頻度と感受性を継続してモニターすることは意味があると思われる。
