褥瘡における細菌保菌:血流感染のリスクの評価および患者の転帰への影響

背景
褥瘡は入院患者、医療従事者、および社会にとって重大な問題である。
目的
多剤耐性微生物のリザーバ、菌血症のリスク因子、および不良予後の予測因子としての入院患者の褥瘡の影響を評価すること。
方法
3 次急性期大学病院に 2005 年 4 月から 12 月および 2009 年 8 月から 2010 年 4 月の 2 期間に 48 時間を超えて入院したステージ II 以上の褥瘡を有する患者を対象として、想定される病院感染病原体および/または多剤耐性病院感染病原体の保菌・感染を評価するために、前向きコホート研究を実施した。
結果
ステージ II 以上の褥瘡を有する合計 145 例の患者を対象とした。このうち 76.5%(145 例中 111 例)の褥瘡に、黄色ブドウ球菌(Staphylococcus aureus)(20.7%)、グラム陰性桿菌(32.5%)、またはその両方(46.8%)の保菌・感染が認められ、その半数以上(64.8%)は多剤耐性病原体であった。患者の50.5%(111 例中 56 例)に菌血症が認められた。菌血症エピソードの 53.6%(56 例中 30 例)では、褥瘡が菌血症の原因である可能性が最も高かった。抗菌薬投与歴(P = 0.04)および感染創(P < 0.001)が、血流感染および 30 日死亡率高値に関連する独立変数であった。30 日死亡のリスク因子は ICU 入室(P = 0.03)および人工呼吸器使用(P = 0.05)などであった。
結論
今回の結果から、褥瘡は多剤耐性病原体の重要なリザーバであることのほか、褥瘡を有する患者は転帰不良な菌血症の高リスク集団であることが示唆される。広域スペクトルの抗菌薬投与歴および感染創の存在は、患者の菌血症と死亡の両方を予測する独立因子であった。
