Active surveillance for asymptomatic carriers of carbapenemase-producing Klebsiella pneumoniae in a hospital setting

カルバペネマーゼ産生肺炎桿菌（carbapenemase-producing Klebsiella pneumoniae；CPKP）の蔓延は、解決困難な公衆衛生上の脅威である。感染・保菌者の早期の特定と隔離が制御対策の鍵となる。本研究では、イタリアの 3 次病院の CPKP スクリーニング戦略について述べる。5 か月の研究期間中に、患者 1,687 例の直腸スワブによるスクリーニングを実施した。このうち 65 例（3.9％）が CPKP 陽性であり、また症例接触者の 5.1％が陽性であった。症例接触者のスクリーニングは、無症状の CPKP 保菌者の検出のためのサーベイランスに不可欠であると考えられる。入院時の選択的 CPKP スクリーニングの意義の大きさは、入院患者中の保菌者の割合に依存する。

445 床の 3 次病院で 5か月間に 1,687 人、3,468 検体の直腸スワブを実施した研究であり、症例が探知されれば、同じ看護ユニットにいた患者を高リスク群としてスクリーニングすることの有用性が示された。入院患者中の保菌者の割合が高ければ、継続して実施することは有用であろうが、そのためにはコストと人手が必要となる。
