Impact of acute kidney injury on mortality and medical costs in patients with meticillin-resistant Staphylococcus aureus bacteraemia: a retrospective, multicentre observational study

背景
メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）感染症に関連する腎障害がその治療中に高頻度に発生するにもかかわらず、腎障害が臨床的・経済的アウトカムに及ぼす有害な影響については評価されていない。
目的
MRSA 菌血症の臨床的および経済的負荷、ならびに治療中に発現した急性腎障害のアウトカムへの影響について評価すること。
方法
MRSA 菌血症のため韓国の病院 8 施設に 2010 年 3 月から 2011 年 2 月に入院した患者のカルテを後向きに調査し、急性腎障害および死亡のリスク因子を評価した。使用した医療資源に基づいて、治療中の MRSA 菌血症患者 1 例あたりの直接医療費を推計した。
結果
合計 335 例の患者が MRSA 菌血症であることが確認された。このうち 135 例（40.3％）に、ファーストラインの抗菌薬療法中に急性腎障害が発現した。急性腎障害の独立リスク因子は、男性、腎臓病の基礎疾患、腹腔内および中心静脈カテーテル感染、および Pitt 菌血症スコア上昇であった。試験期間中に 77 例（23.0％）が死亡した。死亡の独立リスク因子は、固形腫瘍の基礎疾患、Pitt 菌血症スコア高値、および急性腎障害の発現であった。MRSA 菌血症患者 1 例あたりの平均総医療費は 5,435,361 ウォン（4,906 米ドル）と推計され、急性腎障害の発現および ICU 入室が直接医療費の増加の独立予測因子として特定された。急性腎障害の患者では、ベースラインの腎機能を保持していた患者と比較して医療費が 45％増加した。
結論
急性腎障害を発現した患者は、ベースラインの腎機能を保持していた患者と比較して死亡率が有意に高く、直接医療費が高額であった。
