Meticillin-resistant Staphylococcus aureus in elderly residents of care homes: colonization rates and molecular epidemiology

背景
メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）は医療環境および市中環境で死亡と疾患の重要な原因となっている。しかし、介護施設での MRSA 発生を監視した大規模な縦断研究はわずかである。
目的
介護施設の高齢入居者が保菌する MRSA の分子疫学を明らかにすること。
方法
英国・リーズの 65 の介護施設入居者を対象として、連続 4 年間（2006 年から 2009 年）、MRSA 鼻腔内保菌のスクリーニングを行った。分離株の特性を、抗菌薬感受性試験、Pantone-Valentine 型ロイコシジン（PVL）遺伝子座検出、agr アレル判定、ブドウ球菌カセット染色体（SCC）mec タイピング、spa タイピング、およびパルスフィールド・ゲル電気泳動法により評価した。
結果
MRSA は入居者 2,492 名の 888 の鼻腔スワブから回収され、保菌率は研究期間を通して同程度であった（19％から 22％）。3 種類以上の抗菌薬クラスに耐性を示すものが多く（34％）、β-ラクタム系抗菌薬のみに耐性を示すものはまれであった（3％）。PVL 陽性の分離株は同定されなかった。大半の分離株は医療関連の MRSA 流行菌株 15 型（EMRSA-15、ST22-IV）に属していたが、その割合は研究期間中に減少した（86％から 72％へ、P ＜ 0.0001、χ² 検定）。その他の分離株は、複数部位塩基配列タイピング（MLST）による 5 種類の clonal complex（CC）に属していた。最も注目すべきこととして、CC59 株の割合は増加し（10％から 25％へ、P ＜ 0.0001、χ² 検定）、これらの株はムピロシン高度耐性との関連がみられた。
結論
介護施設における MRSA の分子疫学は複雑かつ動的である。MRSA 鼻腔内保菌については、保菌率は一貫して高く、医療関連株が優勢であった。しかし、研究期間中にムピロシン高度耐性との関連を示す 1 クローン群（CC59）の割合が有意に増加しており、警戒を要する。
