一般的な超清浄垂直層流型ユニットの面積は、大手術に必要なすべての器具を収容するには不十分である。従来の層流ユニット本体への固定式超清浄層流装置付き器械台の付設を評価し、ユニット本体に対する物理的・細菌学的影響を判定した。第1期の試験として調査者2名による、患者を配置しない状態での送煙検査を実施したところ、器械台からユニット本体への空気の流入はないことが示され、粒子数の測定からは層流ユニット本体への有害な影響は認められなかった。第2期の試験では患者2例に対する人工膝関節全置換術時に、層流ユニット本体に層流装置付き器械台および層流装置が付いていない器械台を付設した。層流ユニット本体内では、追加で配置した1名の手術室看護師がこれらの器械台で作業を行い、活発な器具操作をシミュレートした。その操作中の粒子径が5 μmを超える粒子数は、層流装置付き器械台では275/m3、層流装置が付いていない器械台では8,550/m3であった(P < 0.0001)。また、手術室看護師が作業を行わない場合は、ユニット本体内部および層流装置付き器械台に隣接する部位の粒子数は有意に減少した(P < 0.03~0.003)。層流装置付き器械台の上およびユニット本体内に設置した平板培地で落下法により測定した細菌数は、それぞれ22 cfu/m2/時、25 cfu/m2/時であったのに対し、層流装置が付いていない器械台では45 cfu/m2/時であった。結論として、送煙検査、粒子数、および細菌学的評価から、層流装置付き器械台の付設は有効であり、手術室の層流ユニット本体を拡張する手法として安全な使用が可能であることが示された。

監訳者コメント:
手術時の層流形成は重要であるが、術者が視覚的にその領域を日常的に認識できないことが問題であろう。
