Three years’ experience of screening for meticillin-resistant Staphylococcus aureus in obstetrics

産科の患者は概ね健常者集団であり、一般的な入院患者集団と比較してメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）リスクが低いと考えられるが、産科を対象とした MRSA スクリーニングに関するデータはほとんどない。2009 年 1 月から 2011 年 12 月に Birmingham Women’s Hospital で出産した女性 21,770 例中 5,548 例（25.5％）を対象として、鼻腔スワブのスクリーニングを実施した。MRSA 陽性は 29 例（0.5％）のみであり、その後 3 例に MRSA 感染症が発生した。さらに、母親が MRSA スクリーニング陰性であった 6 組を含む 13 組の母子に MRSA 感染症が発生した。母子のいずれかが MRSA 陽性であった母 42 例中 17 例が MRSA のリスク因子を有していた。産科では MRSA の蔓延は認められず、この集団を対象とした MRSA 関連疾患の予防のための鼻腔スワブの大規模スクリーニングの価値はわずかである。

監訳者コメント：
母集団として妊婦を長期管理入院例などに限定すると、保菌リスクは高くなることが推測されるが、一般的な分娩でのリスクは筆者が述べているように低い。
