米国における医療従事者の手袋とガウンへのカルバペネム耐性腸内細菌目細菌(carbapenem-resistant Enterobacterales)伝播のリスク因子
Risk factors for transmission of carbapenem-resistant Enterobacterales to healthcare personnel gloves and gowns in the USA

背景
病院はカルバペネム耐性腸内細菌目細菌(carbapenem-resistant Enterobacterales;CRE)の獲得源であり、医療従事者の手指および衣類の汚染は抗菌薬耐性菌の患者間伝播の主な原因であると考えられる。
目的
本研究の目的は、病院においてどの種類の医療従事者が、どの医療従事者と患者の相互作用が、およびどの患者特性が、医療従事者の手袋とガウンへの CRE のより多くの伝播に関連しているかを決定することであった。
方法
本研究は米国の 4 州の病院 5 施設において最近の調査または臨床培養で CRE 陽性だった患者を登録した前向き観察コホート研究であった。医療従事者の手袋とガウンは患者ケア後に培養された。細菌負荷を評価するため、患者の糞便、肛門周辺部、胸部と腕の皮膚からもサンプル採取を行った。
結果
313 例の CRE 保菌患者および患者ケア後に得られた 3,070 の手袋およびガウンの培養において、医療従事者の手袋とガウンはそれぞれ 7.9%、4.3%CRE に汚染されていることが分かった。手袋またはガウンの汚染は相互作用の 10.0%で発生した。汚染は呼吸療法士において最も頻度が高く(15.3%、オッズ比[OR]3.79、95%信頼区間[CI]1.61 ~ 8.94)、またあらゆる医療従事者が患者に触れた時に最も頻度が高かった(OR 1.52、95%CI 1.10 ~ 2.12)。医療従事者の手袋またはガウンへの CRE の伝播との関連性は、集中治療室への在室、臨床培養陽性であること、患者の細菌負荷の増加との間においても認められた。
結論
医療従事者の手袋およびガウンへの CRE の伝播は頻繁に発生した。これらの知見によって、どういった状況とどの患者に対する接触予防策が最も重要であるかについてのエビデンスベースの方針を情報提供できるかもしれない。
監訳者コメント:医療従事者は、自身が汚染することを防ぐためにPPEを使用している。CREを有する患者のケアに使用した手袋、ガウンが汚染されることは当然のことと思われるが、この汚染が、正確なPPEの着脱により、次の患者に伝播することが確実に防がれている必要がある。
