Whole genome sequencing in the prevention and control of Staphylococcus aureus infection

背景
黄色ブドウ球菌（Staphylococcus aureus）は依然として病院感染の主要な起因菌であるが、現在のタイピング法には固有の弱点があり、効果的な感染予防・感染制御の妨げとなっている。全ゲノムシークエンス法は解析能が高く、黄色ブドウ球菌感染に対する我々の理解や管理に画期的な変化をもたらす可能性がある。
目的
全ゲノムシークエンス法の実際について概説するとともに、感染制御実践における今後の可能性を考察すること。
方法
従来のタイピング法を精査し、全ゲノムシークエンス法の潜在的有用性と比較する。
結果
全ゲノムシークエンス法が従来の方法と異なる点として、一塩基の違いまでを識別できること、伝播やアウトブレイクの正確な特性を明らかにできること、さらに抗菌薬感受性や病原性などの表現型の特性における遺伝子的背景に関する情報を得られることが挙げられる。しかし、そのような潜在的有用性を日常業務に生かすことができるかどうかは、コストや所要時間の妥当性、および信頼性の高い標準化されたバイオインフォマティクス基盤の有無によって左右される。
結論
全ゲノムシークエンス法は、アウトブレイク調査を支援し、新興菌株の検出を可能とし、またその臨床的重要性を予測するための汎用性の高い検査法となる可能性がある。
