ドイツの病院における病原性細菌の機械的ベクターとしてのオオチョウバエ(Clogmia albipunctata、ハエ目チョウバエ科)の役割

背景
オオチョウバエ(Clogmia albipunctata、ハエ目チョウバエ科)は以前は地中海沿岸のチョウバエであったが、現在はドイツにも生息しており、病院の建物内で 1 年中みることができる害虫である。
目的
オオチョウバエは、生息しているドイツの病院内で、病原細菌を運搬・伝播する能力を有しているかどうかを調べること。
方法
2011 年 6 月から 2012 年 5 月に、オオチョウバエの成虫 271 匹を、生息している 4 病院で採集し、細菌定着の定性的分析および一部では定量的分析を行った。選択した院内病原菌の抗菌薬感受性試験を実施した。
結果
40 属 45 種の細菌がオオチョウバエに定着していた。分離菌と定着率は、アシネトバクター・バウマニー(Acinetobacter baumannii)0% ~ 17.5%、アエロモナス・ハイドロフィラ(Aeromonas hydrophila)0% ~ 16.7%、アルカリゲネス・フェーカリス(Alcaligenes faecalis)0% ~ 12.5%、バチルス・セレウス(Bacillus cereus)0% ~ 62.1%、大腸菌(Escherichia coli)0% ~ 2.5%、肺炎桿菌(Klebsiella pneumoniae ssp. Pneumoniae)0% ~ 4.1%、緑膿菌(Pseudomonas aeruginosa)0% ~ 12.5%、シュードモナス・フルオレッセンス(P. fluorescens)0% ~ 7.6%、およびステノトロフォモナス・マルトフィリア(Stenotrophomonas maltophilia)0% ~ 10%などであった。さらに、Yersinia frederiksenii(エルシニア・フレデリクセニー)およびノカルジア(Nocardia)属菌が 1 株ずつ検出された。多剤耐性 S. maltophilia 11 株が 1 病院で回収されたのに対し、多剤耐性腸内細菌科細菌は分離されなかった。Acinetobacter 属菌のオオチョウバエへの定着率は 2.9% ~ 36.8%であり、外骨格への高い親和性が示され、A. baumannii ではチョウバエ 1 匹あたり最大 2,080 コロニー形成単位であった。
結論
オオチョウバエは、院内感染と関連する病原性細菌の機械的ベクターである可能性がある。

監訳者コメント:
感染症の運び屋としての蚊やハエは、常に衛生管理上の課題である。古典的ともいえる方法で、こうした地道な研究を試みることには頭が下がる。一方で、こうしたハエよりも医療環境で人が運ぶ病原菌のほうが菌種も菌量もはるかに多く、医療関連感染への影響力を考えると比でないのではないだろうか。
