Multidrug-resistant bacteria in travellers hospitalized abroad: prevalence, characteristics, and influence on clinical outcome

背景
世界的に多剤耐性菌（MDR）の負荷は増大しており、特に病院環境で顕著である。
目的
海外の病院から転院した旅行者から検出された MDR の特性および臨床的重要性を調べること。
方法
この後向き研究の対象は、海外の病院からスイスの University Hospital Zurich に転院し、メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）、基質特異性拡張型β-ラクタマーゼ産生菌（ESBL）、および多剤耐性グラム陰性菌の保菌について通常の入院時スクリーニングを受けた患者とした。
結果
259 例中 46 例（17％）に MDR の保菌、9 例（3.5％）に感染が認められた。33 例（12％）が 1 菌種、12 例（4.6％）が 2 菌種、3 例（1.2％）が 3 菌種を保菌していた。合計で ESBL 36株、多剤耐性グラム陰性菌 21 株、MRSA 3 株の分離株が検出された。分離頻度が高かった菌種は、大腸菌（Escherichia coli）（18 株、30％）、肺炎桿菌（Klebsiella pneumoniae）（14 株、23％）、およびアシネトバクター・バウマニー（Acinetobacter baumannii）（14 株、23％）であった。検出部位で多かったのは、皮膚（97％）および気道（41％）であった。保菌がある場合は ICU 入室期間が長期にわたった（中央値［範囲］ 8［1 ～ 35］対 3.5［1 ～ 78］日、P ＝ 0.011）。院内死亡率は、MDR 保菌患者（10.9％）のほうが非保菌患者（2.3％）よりも高かった（P ＝ 0.018）。MDR 保菌は死亡と関連していた（補正オッズ比 5.176、95％信頼区間 1.325 ～ 20.218）。
結論
海外から転院した患者には高い割合で MDR 保菌が認められ、保菌は不良な臨床転帰と関連していた。
