Invasive aspergillosis: drug-dispensing systems as a source of filamentous fungal contamination in high-risk units?

Grenoble University Hospital の空気感染制御を実施している高リスク病棟には自動薬剤搬送装置が設置されており、薬剤部から病棟まで薬剤輸送カートによって薬剤が搬送され、毎日補充されている。本研究の目的は、輸送カート内の糸状真菌汚染レベルを、通常の条件下および Aniosurf^®（殺真菌消毒薬）による洗浄後に評価することである。糸状真菌は全サンプル中から検出され、サンプルの 83.3％はアスペルギルス・フミガーツス（Aspergillus fumigatus）で汚染されていた。輸送カートを Aniosurf^® で洗浄したところ、糸状真菌のレベルが有意に減少したが、非制御環境下での 24 時間の保管後には再び汚染が認められた。

監訳者コメント：
ショッキングなタイトルの論文である。対象となった血液内科病棟は、HEPA フィルタを設置し、陽圧管理されている。この病棟内に搬送されるカートに糸状菌が付着していたという。改修工事などで空気中に巻き上げられたアスペルギルスを吸引することでアウトブレイクの原因となるという報告はあるが、カートに付着しているアスペルギルスが感染源としてリスクとなり得るのか、そもそも費用と人力に限りのある感染対策は、リスクアセスメントを行い、優先順位を付けて実施したい。
