術後感染症の症例対照研究におけるマッチング基準★

Matching criteria in case-control studies on postoperative infections

症例対照研究では一般に、主要な交絡因子の影響を補正するためにマッチングを行う。本研究では、マッチングが行われる頻度、およびその特異的な因子を明らかにするために、術後感染症に関する症例対照研究に関する入手可能なエビデンスを評価した。2006年8月までの該当する症例対照研究をPubMedで検索し、さらに個々の症例と対照とのマッチングを実施している研究の評価を行った。該当する研究は42件であり、これらを評価した。年齢をマッチングの基準としていた症例対照研究は、42件中27件(64.3%)であった。2番目に多く使用されていた基準は特定の手術手技であり、42件中17件(40.5%)であった。性別は、症例と対照患者のマッチング基準として42件中14件(33.3%)の研究で使用された。手術部位感染および/または他の術後感染症の発症リスク期間、すなわち手術から感染の診断までの期間は42件中9件(21.4%)の研究で使用され、手術日と、症例および対照患者に手術が必要となった主診断についても同様であった。外科医または外科チームが同一であることが7件の研究に使用されており(16.7%)、全米病院感染サーベイランス(NNIS)システムのリスクスコアに従ったマッチングは5件の研究で使用された(11.9%)。今回の解析の結果から、術後感染症の症例対照研究のマッチングでは様々な特性が使用されていることが示された。特定の手術手技によるマッチングをより多くの研究で使用することは、この臨床研究領域における症例対照研究の内的妥当性の向上に役立つ可能性がある。

監訳者コメント:
手術部位感染のリスク評価を行う場合に、どのような因子をそろえ、どのような因子を比較の対象として考慮すればよいかを考える上で、助けになる論文である。結論としては、手術手技は最低でもそろえたほうがよいということであるが、外科学の観点からは比較的妥当である。
