Priorities in the prevention and control of multidrug-resistant Enterobacteriaceae in hospitals

背景
多剤耐性腸内細菌科細菌（MDE）は世界的に蔓延しており、治療の選択肢が少ないことから、公衆衛生上の重大な脅威となっている。さらに、メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）とは異なり、MDE には複数の属の菌が含まれ、また基質特異性拡張型β-ラクタマーゼやカルバペネマーゼなどの種々の耐性機序が存在することが、通常の診断検査室での検出を困難にしている。国内外の公式なガイドラインがないため、現在の多くの病院の封じ込め対策は、やや場当たり的なものとなりがちである。
目的
MDE の予防・制御において何を優先すべきか、また何が実行可能かを明らかにすることを目的とした。また、今後の研究が必要な領域を明確にすることとした。
方法
MDE の制御のために用いられた方策や介入に関する公表論文や国の機関などによる資料のレビューを実施した。
結果
特に集中治療の領域のように特定のカテゴリのリスク患者を対象として適切な臨床検査法によるスクリーニングを行うべきである。標準予防策および接触予防策は不可欠であり、手指衛生遵守を継続して強調し、高い遵守率を保つ必要がある。MDE は環境表面に何週間にもわたって残存する可能性があるため、環境の汚染除去も有効な制御介入となり得る。現時点では除菌の選択肢は限られており、研究も不十分であるため、病院内外での抗菌薬の適正使用が依然として重要である。
結論
予防・制御の指針となるエビデンスが明らかに不足しているため、例えば迅速な検出など、重要な領域の研究が緊要である。
