Risk factors for mortality in Clostridium difficile infection in the general hospital population: a systematic review

背景
クロストリジウム・ディフィシル（Clostridium difficile）感染症（CDI）は最も重大な医療関連感染症の 1 つであり、多くの死亡をもたらしている。死亡リスクを有する CDI 患者を特定するための種々の重症度スコアが提唱されているが、これらの指標の基盤となっているエビデンスに関するシステマティックレビューは発表されていない。このようなレビューによって、死亡を良好に予測するスコアの開発が可能になると考えられる。
目的
臨床的に有用な CDI による死亡のリスク指標を検討した公表文献のシステマティックレビュー。
方法
MEDLINE（1950 年から現在）、Web of Science の conference proceedings（1899 年から現在）、および BIOSIS Citation Index（1969 年から現在）を、PubMed および Web of Knowledge を用いて検索した。リスク指標候補として、4 件以上の研究が評価対象としていた指標を抽出した。
結果
最初に特定した 1,617 件の研究中 26 件が選択基準に合致していた。そのほとんどは後向きコホート研究であり、半数以上は米国で実施されたものであった。死亡との関連が高い頻度で指摘されていたリスク指標は、高齢、白血球数高値、クレアチニン高値、アルブミン低値であり、やや少ないところではステロイド薬の使用であった。発熱、ヘモグロビン/ヘマトクリット値、下痢の重症度、腎疾患、糖尿病、癌、または経鼻胃管の使用は、死亡とは関連ないと考えられた。
結論
本研究の結果から、CDI による死亡の有用なリスク指標として年齢、白血球数、血清クレアチニン、血清アルブミン、およびおそらくはステロイド薬投与歴を用いるべきであることが支持された。発熱、ヘモグロビン/ヘマトクリット値、下痢の重症度、およびいくつかの併存疾患を有用なリスク指標として使用することは支持されず、CDI の重症度スコアにこれらを組み入れることは疑問視された。
