Investigation of hospital discharge cases and SARS-CoV-2 introduction into Lothian care homes

背景

スコットランドにおける重症急性呼吸器症候群コロナウイルス- 2（SARS-CoV-2）の流行の第一波は、介護施設の症例数および死亡率の増加をもたらした。ロージアンの介護施設の 3 分の 1 以上でアウトブレイクが報告されたが、介護施設に退院した入院患者の検査は限られていた。

目的

流行の第一波時に病院から退院した患者を、介護施設への SARS-CoV-2 の持ち込み源として調査すること。

方法

2020 年 3 月 1 日から 2020 年 5 月 31 日までに病院から介護施設に退院したすべての患者を対象に臨床レビューを実施した。COVID-19 の検査歴、退院時の臨床評価、全ゲノムシークエンシング（WGS）データ、および 14 日の感染期間に基づいて、エピソードを除外した。WGS 解析のために臨床サンプルを処理し、構築されたコンセンサスゲノムを、Cluster Investigation and Virus Epidemiological Tool ソフトウェアによる解析に使用した。電子入院記録により患者のタイムラインを入手した。

結果

病院から介護施設に退院した患者計 787 例を特定した。このうち 776 例（99％）は、介護施設への SARS-CoV-2 がその後の持ち込みであったため除外された。一方、10 件の退院エピソードについては、コンセンサスゲノムのゲノムの多様性が低い状態、あるいはシークエンシングデータが入手不可能であったため、結果は不確定であった。入院期間にゲノム、時間、および場所が陽性症例に関連付けられた退院エピソードは 1 件のみで、その介護施設の陽性症例 10 例につながった。

結論

病院から退院した患者の大多数が、介護施設への SARS-CoV-2 の持ち込みの可能性を除外された。このことは、新興ウイルスに直面し、ワクチンが入手できない場合、すべての新規入院をスクリーニングすることの重要性を強調するものである。

監訳者コメント：

介護施設のクラスター発生の原因として、病院からの入所は稀であることを示し、またそのため病院からの入所以外の入所者を対象とした施設入所時のスクリーニング検査が重要であると報告している。

病院からの入所者が感染していたら、そもそも病院でクラスターが起きているだろうし、感覚的にも個の論文の結果に一致した実感はある。
