Effect of an infection control programme on bacterial contamination of enteral feed in nursing homes

背景
ナーシングホーム職員の経腸栄養の実践にかかわる感染制御の知識が不十分であると、経腸栄養システムの汚染をもたらす可能性がある。
目的
ナーシングホーム職員の経腸栄養の知識と実践に対する感染制御プログラムの効果を調べること、および感染制御プログラムと職員の手指および経腸栄養器具の細菌汚染との関連を評価すること。
方法
準実験的pre-test–post-test control designを用い、いずれも入居者 15 名と職員 10 名で構成される介入群および対照群を設定した。経腸栄養の知識と実践を質問票と観察により評価した。介入群に対しては感染制御プログラムを実施した。職員の手指、経腸栄養、流量制御装置、栄養チューブのハブ、および入居者の鼻咽頭と胃液からサンプルを採取し、感染制御プログラム導入前後の全細菌数およびメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）の有無を調べた。
結果
介入群では感染制御プログラム実施後、知識と実践が有意な改善を示した（P ＜ 0.05）。職員の手指、チューブハブ、および入居者の鼻咽頭と胃液から採取した pre-test サンプルには 10⁴ コロニー形成単位（cfu）/mL を超える汚染が認められた。介入群では post-test サンプルの汚染は有意に減少したが（P ＜ 0.05）、対照群では変化はなかった。介入群の MRSA 陽性サンプル数の平均値（± 標準偏差）は 2.1 ± 1.6 から 0.4 ± 0.7（P ＜ 0.05）に減少した。MRSA による手指汚染は、制御装置、胃液、チューブハブ、および経腸栄養の汚染と高い相関があり（P ＜ 0.05）、経腸栄養の汚染と不良な手指衛生との密接な関連が示唆された。
結論
効果的な感染制御プログラムにより、経腸栄養の細菌汚染を大幅に減少させることが可能である。ナーシングホームに感染制御プログラムを導入することが強く推奨される。
