Healthcare workers’ perceptions of occupational exposure to blood-borne viruses and reporting barriers: a questionnaire-based study

背景
医療従事者は血液媒介ウイルスへの曝露リスクが極めて高い。
目的
血液媒介ウイルスへの職業曝露、およびその事故報告に関与すると考えられる障壁について、医療従事者の認識を調査すること。
方法
King’s College Hospital NHS Foundation TrustのDental Instituteに勤務する合計 120 名の医療従事者が多施設研究の一環として無記名の質問票に記入した。
結論
回答者の 86％（99/115）は業務中の血液媒介ウイルス感染を懸念していた。C 型肝炎ウイルス（HCV）が最大の脅威であるとした回答者の 69％（31/45）は、HCV が自身の健康に対するリスクが最も高いと考えていたが、B型肝炎ウイルスの場合は 53％（10/19）、ヒト免疫不全ウイルス（HIV）では 13％（5/40）であった（いずれもP ＜ 0.001）。医療従事者としての経験年数が 21 年以上の回答者の 75％（18/24）は HIV 伝播のリスクを認識していたが、5 年未満の回答者では 13％（2/16）であった（P ＝ 0.002）。経験年数 21 年以上の回答者は全員（23/23）が HIV 曝露後予防投与について知っていたが、21 年未満の回答者では 20％（12/60）であった。回答者の 92％（104/113）はすべての体液曝露事故を報告することの重要性に同意したが、自身の曝露事故をすべて報告していたのは 58％（28/48）のみであった。電子報告システムによりこのような事故報告が改善されるであろうということに同意したのは 59％（60/102）であった。
結論
本研究から、血液媒介ウイルスの感染リスクとその管理に関する医療従事者の知識を改善する必要があることが明らかとなった。本研究で収集したデータは、英国の血液媒介ウイルス職業曝露に関するウェブベースのサーベイランスシステム構築のために提供される予定である。
