Community-associated meticillin-resistant Staphylococcus aureus: the case for a genotypic definition

背景
新しい市中獲得型メチシリン耐性黄色ブドウ球菌（community-associated meticillin-resistant Staphylococcus aureus；CA-MRSA）株が、市中の健常者における感染症の原因菌として拡大している。CA-MRSA の同定は、臨床的管理、疫学分析、感染予防・管理、および規制当局への報告を行ううえで重要であるが、これらの株の定義や命名には混乱がある。
目的
従来の CA-MRSA の定義をレビューするとともに、新しい定義を提案すること。
方法
非システマティックレビュー。
結果
これまでは、CA-MRSA と医療関連メチシリン耐性黄色ブドウ球菌（healthcare-associated meticillin-resistant S. aureus；HA-MRSA）株の識別には疫学的定義が有用であった。しかし、HA-MRSA 株は市中ではほとんど伝播していないのに対して、CA-MRSA は医療施設での伝播がみられ始めており、疫学的定義は無効になりつつある。CA-MRSA は、メチシリン耐性遺伝子 mecA を獲得した黄色ブドウ球菌市中株である。これらの株は HA-MRSA とは異なるものであり、遺伝的に定義すべきである。このような定義は、複数部位塩基配列タイピング（MLST）または spa タイピングによる遺伝子型決定と、ブドウ球菌カセット染色体（SCC）mec の解析との併用により可能になると考えられる。Pantone-Valentine 型ロイコシジンの有無や抗菌薬感受性プロファイルは、CA-MRSA の有用な指標となり得るが、それらの定義に使用するべきではない。
結論
MRSA 感染の疫学を十分に評価するためには、MRSA 感染を（1）HA-MRSA 株または CA-MRSA 株により引き起こされたもの、（2）市中または医療施設で獲得されたもの、および（3）市中または医療施設で発現したもの、という特性に基づいて分類を行うべきである。

院内でもアウトブレイクする可能性のある市中獲得型メチシリン耐性黄色ブドウ球菌の定義づけと監視が先進国で課題となっている。院内で日常的に実現できる検査法ではない手法で型別しても、なかなか現場に受け入れられないのが課題である。
