Improving implementation of infection control guidelines to reduce nosocomial infection rates: pioneering the report card

背景
公表されている感染制御ガイドラインに基づいて、病院の各診療科および手術室の感染制御専門家が日常的に使用するための 2 種類の詳細なチェックリストを開発した。
目的
病院 3 施設を対象として、院内感染率に対するチェックリストの効果を 1 年間にわたって評価すること。
方法
チェックリストは 20 の小見出しからなる（約 150 項目）。当プロジェクトの看護師が介入対象の診療科を巡回した（対照の診療科には巡回なし）。巡回の際に、看護師は 15 から 20 の観察項目を選択し、観察した行動の適切性をチェックリストに記録し、その場で改善指導を行った。チェックリストの遵守率、抗菌薬使用状況、培養実施数と培養陽性数、および職員の手指と患者の周囲環境の培養結果を、報告カードを用いて関係者と管理者に毎月 1 回報告した。院内感染率を最初の月と最後の月に評価した。
結果
ベースライン時の院内感染率は、介入対象の診療科 11％（37/345）、対照の診療科 10％（26/270）であり、同等であった。最後の月の院内感染率は、介入対象の診療科では 4％（16/383）に低下したが（P ＜ 0.01）、対照の診療科では 8％（21/248）にわずかに低下したのみであった（有意差なし）。培養実施数、培養陽性数、抗菌薬使用に顕著な傾向は認められなかった。ガイドライン遵守率は病院間で 75％から 94％の範囲であり（P ＜ 0.001）、全遵守率は 80％から 91％に上昇した（P ＜ 0.01）。
結論
毎月 1 回の報告と、感染制御の巡回実施時のチェックリスト使用を併用することにより、介入対象の診療科の院内感染が有意に減少した。

監訳者コメント：
チェックリストは 2006 年ごろから米国で浸透してきた。とうとう英国でも市民権を得た形だ。日本での運用も真剣に考えるときがきた。
