背景
人工呼吸器関連肺炎(VAP)は集中治療における重大な問題であり、適切なバイオマーカーが強く求められている。プロカルシトニン(PCT)がマーカー候補として提案されている。
目的
VAP を早期発症した非外科患者に対する PCT 濃度モニタリングの臨床的有用性を評価すること。
方法
早期発症 VAP(定義は、人工呼吸器の装着後 48 時間 から 6 日で発症し、診断された VAP)患者 34 例を登録した。血清 PCT レベルを 1、2、3、5、6、および 7 日目に測定した。
結果
死亡率は 21%であった。非生存者では、3 および 7 日目の PCT レベルが有意に上昇していた。非生存者の PCT の受信者動作特性曲線下面積(AUC)は、3 日目 0.762(95%信頼区間[CI] 0.6 ~ 0.923)、7 日目 0.754(95%CI 0.586 ~ 0.922)であった。敗血症患者では、PCT は 1、2、3、5、および 7 日目に有意に高く、AUC は 1 日目が最大(0.783、95%CI 0.626 ~ 0.94)であった。1 日目のカットオフ値を 1 ng/mL とした場合の敗血症性ショック発症の陽性適中率は 0.813 であった。
結論
PCT 濃度と抗菌薬療法の妥当性または VAP の原因菌との間に関連は認められなかった。ロジスティック回帰分析から、PCT と不良な転帰との有意な相関はみられなかった。非生存者および敗血症性ショック発症患者のほうが PCT レベルが高かったが、PCT はこれらの転帰の強力な予測因子ではない。

監訳者コメント:
PCT はカルシトニンの前駆物質であり、細菌や真菌による感染症では、炎症性サイトカインの刺激により全身の臓器で PCT が産生されることから、細菌感染症のマーカーとしての有用性が多くの論文で検討されている。この論文では、PCT が VAP の診断や予後の予測に使えるかを検討したが、近年の他の感染症における検討と同様に、明確な相関は期待できなかった。
