Pre-operative skin preparation practices: results of the 2007 French national assessment

背景
内因性微生物叢の減少を目的とした術前皮膚処置は、手術部位感染リスクの低下のために実施される主要な予防法の 1 つである。フランスでは、感染リスクの術前管理に関する国からの勧告が 2004 年に公表された。
目的
術前皮膚処置に関するフランスのナショナル・ガイドラインの遵守について、2007 年に評価を実施すること。
方法
フランスの病院で、患者と職員への面接および病院業務の観察による前向き監査を実施した。ガイドラインから下記の 5 つの主要な基準を選択し、その遵守について調査した：患者への情報提供、術前シャワー、術前除毛、手術部位消毒、およびこれらの処置の記録。
結果
609 施設の全診療科から得た 41,188 例の患者データを分析した。患者の 88.2％（95％信頼区間［CI］87.9 ～ 88.5）では、術前シャワーについての情報が患者に提供されていた。洗髪を含む、抗菌性の皮膚洗浄液を用いた術前シャワーの推奨手順には患者の 70.3％（95％CI 69.9 ～ 70.8）が従っていた。患者が適切な情報を提供されている場合は、この遵守率は上昇した（P ＜ 0.001）。毛の処理手順の遵守率は 91.5％（95％CI 91.2 ～ 91.8）であり、手術部位消毒の推奨手順の遵守は 25,529 例（62.0％、95％CI 61.5 ～ 62.5）にみられた。患者への情報提供は患者の 35.6％、術前消毒は 82.3％、術前の手術部位消毒は 71.7％でそれぞれ記録されていた。
結論
フランスのガイドラインの基本的な内容は理解されていると考えられたが、注意喚起は必要である。いくつかの推奨手順については診療科ごとに修正すべきであろう。
