Dermal and pulmonary absorption of ethanol from alcohol-based hand rub

背景
職場で擦式アルコール製剤を使用する医療従事者にとっては、エタノール中毒が深刻な問題となり得る。本研究では、医療従事者の手指消毒後のエタノール吸収レベルを測定した。

方法
Nancy 大学病院の医療従事者 86 名を対象として、4 時間の勤務時間の前後に検査を行った。参加者は勤務中に 70％エタノール含有擦式アルコール製剤を使用した。血中および尿中のエタノール、アセトアルデヒド、および酢酸塩レベルをガスクロマトグラフィーを用いて測定した。酒気検知器により呼気中のエタノールレベルを測定した。

結果
曝露 1 ～ 2 分後に、医療従事者 28 名の呼気中にエタノールが検出された（平均濃度 0.076 mg/L［標準偏差 0.05］）。4 時間の勤務後には、いずれの参加者の血中にもエタノール、アセトアルデヒド、および酢酸塩は検出されず、尿検査も陰性であった。

結論
擦式アルコール製剤由来エタノールの曝露、特に蒸気の吸入により、曝露から 1 ～ 2 分後に酒気検出器は陽性を示した。皮膚からのエタノール吸収は認められなかった。肺からの吸収は認められたが、毒性レベル未満であった。

監訳者コメント：
職業曝露の観点から研究された課題と考えられる。欧米ではn-プロパノールやイソプロパノールが消毒薬として汎用されており、発癌性や刺激性等の長期的な曝露による有害事象も課題となっている。
