Infection control systems in transition: the challenges for post-Soviet Bloc countries

背景
ヨーロッパとアジアの現在の 30 か国は 20 年前、ソビエト連邦支配下または直轄の社会主義政府を擁していた。これらの国々の多くは、集中的な医療制度改革により感染制御の状況が変化している。しかし、これらの文献の多くは、他国の科学者が入手することが困難である。
目的
旧ソビエト連邦および同盟諸国における現行の感染制御方針と実践について整理すること。
方法
PubMed および Google による検索のほか、各国のウェブサイトや灰色文献^※についても調査した。数か国の言語による合計 192 件の文献のレビューを行った。
結果
これらの国の感染制御の状況は過渡期にある。国を 3 グループに分類した。第 1 グループの早期に欧州連合に加盟した東欧諸国およびバルト 3 国は、特定の病原体と抗菌薬使用に関するサーベイランスシステムを構築している。第 2 グループの欧州内のその他の旧同盟諸国では、近年、感染制御のインフラが確立し、これを欧州のサーベイランスプログラムと同調させることに傾注している。第 3 グループの旧ソ連諸国、モンゴル、および紛争後の東欧諸国では、改革の初期段階にある。医療従事者の権限の欠如、医療資源の不足、および専門知識の不足が確認された。感染制御に関する公的な統計の過少報告が広く行われている。
結論
国際的な機関による指導が近代的な感染制御プログラムの開発・導入に極めて重要であると考えられた。第 3 グループの国では感染制御の問題が依然として軽視されており、国際社会からの一層の支援が必要である。
