Infection rates associated with total parenteral nutrition

中心静脈アクセス器材の使用は増加しているが、このような器材には感染リスクが伴う。リスク低下を図るための追加的戦略として、回避可能な感染を減少させるための研究が必要である。完全静脈栄養を受けている患者のカテーテル出口部を透明な 2％グルコン酸クロルヘキシジン含有抗菌ドレッシングで被覆することについて、12 か月間の前向き監査を実施した。その結果、カテーテル関連血流感染症（CRBSI）の症例数が 8 例から 0 例に減少し（P ＝ 0.057）、このフィルムドレッシングは、このような高リスク患者群の回避可能なCRBSIをゼロにするという目標を達成するうえで有用な追加的方策であると考えられた。

監訳者コメント：

カテーテル刺入部にクロルヘキシジン含有のスポンジを使用して被覆することが CRBSI 防止につながると、2011 年に出された CDC の BSI 防止ガイドラインにも明記されている。本研究ではスポンジではなく、ドレッシングにジェル剤を組み込んだ製品が使用されているが、同様に BSI 防止効果があることは先行する研究で示されている。そうした介入の有効性を確認したスタディである。
