Modelling the transmission dynamics of meticillin-resistant Staphylococcus aureus in Beijing Tongren hospital

中国の多くの 3 次病院では、セミプロのボランティアが医療助手として働いている。ボランティアが関与する院内伝播の減少を図るためには、適切な感染制御策が必要である。中国、北京にある北京同仁医院のボランティアを対象として、救急病棟および呼吸器集中治療室におけるメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）の伝播特性を記述するために、コンパートメントモデルを作成した。本モデルでは、対象者を非保菌患者か保菌患者か、非汚染医療従事者か汚染医療従事者か、および非汚染ボランティアか汚染ボランティアかというコンパートメントに分類する。基本再生産数^※（R₀）を算出し、モデルの種々の変数に対する R₀ の依存性を分析した。さらに、2009 年 3 月 3 日から 2010 年 2 月 28 日に救急病棟および呼吸器集中治療室から報告された保菌患者数のデータを比較するために、モデルのシミュレーションを行った。R₀ の感度分析から、医療従事者とボランティアの手洗いの遵守率が上昇すれば、伝播リスクは劇的に低下することが示された。ボランティアは 1 対 1 で患者ケアを行っているため、ボランティアが医療従事者に置き換わると MRSA 陽性患者が増加することが本研究から示された。したがって、医療従事者における手指衛生の改善に加えて、適切な訓練を受けたボランティアを採用することが、病院環境における MRSA および多剤耐性菌感染の減少のための魅力的な代替方法である。
