Perioperative variables associated with surgical site infection in breast cancer surgery

乳房手術は一般的に清潔手技に分類されるものの、手術部位感染症（SSI）発生率は高いことが報告されている。本研究の目的は、乳癌患者の SSI に関連する周術期の因子を特定することである。大学病院で 2005 年 1 月から 2007 年 8 月に手術を受けた乳癌患者のカルテレビューを実施した。患者 199 例から術前、術中、および術後の臨床データを抽出し、解析を行った。全体の SSI 発生率は 19.1％（38 例）であった。SSI は、体格指数（BSI）高値（P ＝ 0.001）、糖尿病既往歴（P ＜ 0.0001）、喫煙（P ＜ 0.0001）、または活動性の皮膚障害（P ＜ 0.0001）と関連していた。SSI と関連するその他の因子は、腫瘍の臨床病期が進行期であること（P ＝ 0.003）および術前療法の施行（P ＝ 0.003）などであった。乳房温存手術は、根治的手術（乳房切除術単独および乳房切除術時の同時再建）と比較して SSI との関連が小さかった（P ＝ 0.0001）。
