Nosocomial acute-onset postoperative endophthalmitis at a university teaching hospital in China

2000 年 1 月 1 日から 2009 年 12 月 30 日に中国の中山眼科センターで実施されたすべての眼内手術を対象とした後向き研究を行い、院内急性発症術後眼内炎の詳細なデータを得た。この期間に合計 147,244 件の眼内手術が実施された。29 例が急性発症術後眼内炎と診断され、その発生率は 0.020％であった。この 10 年間にわたり発生率は低値が持続し、増加は認められなかった。眼内炎の発生率は、眼内レンズの二次移植後が最も高かった（0.129％）。白内障手術後の発生率は 0.01％であり、この値は他の大規模研究での推定値の下限であった。最も頻度が高い分離菌はグラム陽性細菌（71％）であり、大半は表皮ブドウ球菌（Staphylococcus epidermidis）（64％）であった。しかし、表皮ブドウ球菌は白内障摘出術後の患者には認められなかった。これらの患者は、手術終了時にバンコマイシンの前房腔内投与を受けていた。術後眼内炎患者の視力の転帰は全般的に不良であった。経過観察のための最終来院時の視力は 20/40 以上 3 例（10％、全例が白内障手術施行例）、20/400 以下 15 例（52％、10 例は経扁平部硝子体切除術施行例）であった。視力の転帰不良に関連する因子は、初回測定視力が手動弁以下であること、および培養陽性などであった。10 年間の本研究の結果は、他の施設や今後の研究の比較対象として利用できると考えられる。
