クロルヘキシジンによる術前処理の効果の経時的変化

術前予防策や無菌操作技術の進歩は継続しているものの、手術部位感染症は依然として問題となっている。本研究の目的は、一般的な術前処理溶液であるクロルヘキシジンの効果の経時的変化を種々の濃度で評価することである。黄色ブドウ球菌(Staphylococcus aureus)(lux)をミュラーヒントン寒天平板培地に接種した。この細菌は遺伝子組換えにより発光性を示し、光学イメージングシステムを用いて定量が可能である。規定量のクロルヘキシジン水溶液を 3 種類の濃度(第 1 群 4%、第 2 群 2%、第 3 群 0.4%)で寒天平板に添加し、細菌数の減少を比較した。接触時間 2 分後の第 1 群と第 2 群の細菌数減少は同等であり、各群の残存細菌数は 30%であった(P = 0.512)が、第 3 群の細菌数(33%)は第 1 群および第 2 群のいずれとの比較でも有意に多かった(第 1 群対第 3 群でP < 0.0001、第 2 群対第 3 群でP = 0.0002)。全 3 群の細菌数は経時的に減少し、最終時点(1 時間)の残存細菌数は第 1 群が最少の 9%(P < 0.0001)、第 3 群が最多の 19%(P < 0.0001)であった。本研究から 2 つの重要な結果が示された。すなわち、1 つはクロルヘキシジンの希釈率は殺菌効果と直接的に相関すること、もう 1 つはクロルヘキシジンの効果は接触時間と直接的に関連することである。本研究の結果に基づいて、手術部位の処理には 4%クロルヘキシジンを使用すること、および皮膚切開前の接触時間を最低 2 分間とすることを推奨する。
