クロストリジウム・ディフィシル(Clostridium difficile)関連下痢症はこの 15 年間で著しく増加しているが、転帰の予測因子については十分に明らかにされていない。本研究は、2002 年から 2009 年にイングランド北東部で実施された 2,761 例の患者を対象としたコホート研究であり、その評価項目は 30 日死亡率であった。二項ロジスティック回帰により、年齢、性別、および併存疾患の影響を評価した。四捨五入して整数値としたオッズ比を用いて予測スコアを開発した。年齢、腎疾患、および癌に基づく予測スコア(ARC スコア)により、30 日死亡リスクの異なるグループに患者を区分することができた(スコア 0 ~ 3 のリスクは 9% ~ 21%、スコア 4 ~ 7 は 31% ~ 48%、スコア 8 は 66%)。併存疾患は、C. difficile 関連下痢症の転帰の重要な予測因子であることが示された。併存疾患と年齢を組み入れた ARC スコアにより生存確率を評価することが可能である。他の集団でさらなる検証を行う必要があるものの、ARC スコアは診療と研究に重要な意味をもつ。

監訳者コメント

日本ではまだまだ C. difficile 関連下痢症の頻度が低く、転帰についてもその詳細が明らかでない。しかし、本研究で示された、年齢・腎疾患・癌が本症の転帰の予測因子になり得ることは知っておいたほうがよいと思われる。スコアは、年齢 4 点(80 歳以上が最高点)、癌 2 点、腎疾患 2 点であり、最高 8 点である。
