Burden of infections among 44,869 elderly in nursing homes: a cross-sectional cluster nationwide survey

フランスには高齢者用のナーシングホームが 445,000 床あるが、ナーシングホームにおける感染症の負荷に関する研究の発表論文はない。本研究の目的は、入居者の感染症有病率を推計するとともに、感染症と入居者のリスク因子との関連の程度を評価することである。1 か月を 1 期間として 5 期間にわたる全国的な多施設横断的クラスターサーベイにより有病率を調査した。感染症例を「確定例」と、臨床検査による確認のない「ほぼ確実例」に分類した。578 の参加施設の入居者合計 44,869 名を調査対象として登録した。感染症有病率は全体で 11.23％（95％信頼区間［CI］10.50 ～ 11.97）であり、調査期間（調査が実施された年）の間に有意差がみられた（P ＜ 0.001）。「確定例」は 4.60％（95％CI 4.04 ～ 5.54）、「ほぼ確実例」は 6.63％（95％CI 5.77 ～ 7.98）であった。気道感染症が最も頻度が高く、全感染症の 41％を占めていた。感染症と、年齢、入居期間、自立性障害、尿路デバイス、褥瘡、および調査期間との間に有意な関連がみられた。この脆弱な集団に対する感染症の影響を軽減できるのは、予防プログラムのみであると考えられる。
