背景
インフルエンザワクチン接種はインフルエンザ院内伝播を予防するための制御計画の重要な構成要素であり、すべての医療従事者に対して推奨されているにもかかわらず、医療従事者の高いインフルエンザワクチン接種率を達成することは課題となっている。日本では、医療従事者のインフルエンザワクチン接種率に関するデータは限られている。さらに、パンデミック発生年以降の医療従事者のインフルエンザワクチン接種率に対するパンデミックインフルエンザの影響については不明である。
目的
日本の 3 次医療施設の医療従事者のインフルエンザワクチン接種率を明らかにすること、および医療従事者のワクチン接種率を向上させるために、さらに介入が必要であるかどうかを評価すること。
方法
札幌市の 550 床の 3 次医療施設における医療従事者の 2005 年から 2010 年のインフルエンザワクチン接種率を、病院の産業保健サービスデータベースを用いて後向きに調査した。
結果
本集団の季節性インフルエンザワクチン接種率は2005 年から 2010 年にかけて徐々に上昇し、また 2009/2010 年のパンデミックインフルエンザ(H1N1)シーズン中の接種率は高かった。しかし、2010/2011 年の季節性インフルエンザワクチン接種率は、2009/2010 年のパンデミックワクチン接種率と比較して有意に低下し、全医療従事者の中では医師のワクチン接種率が最も低かった。
結論
パンデミックインフルエンザは、後年のインフルエンザワクチン接種に対して持続的な影響を及ぼさないと考えられる。したがって、パンデミック発生年以降も医療従事者のワクチン接種率をモニターする必要があり、また適切なワクチン接種率を維持するためにはワクチン接種義務化プログラムなどの介入を実施することを考慮すべきである。
