Investigation of classical epidemiological links between patients harbouring identical, non-predominant meticillin-resistant Staphylococcus aureus genotypes and lessons for epidemiological tracking

分子疫学理論では、識別能が高い分子タイピング法により 2 つの分離株の類似性が認められた場合、これらは同一の感染鎖に属していると見なす。これについてはアウトブレイクを対象とした研究は行われているが、地域的流行についてはほとんど検討されていない。メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）分離株が地域流行性の優勢な遺伝子型である場合は、ヒトからヒトへの伝播を立証することができない。一方、頻度が低い遺伝子型の分離株のほうが疫学的追跡に適していると考えられる。本研究の目的は、優勢ではない遺伝子型の MRSA を保菌する新規患者を対象として、分子タイピングから推定された疫学的関連は、地域的サーベイランスプログラムにより収集した古典的な疫学データに代わり得るかを検討することである。clfB および spa 遺伝子の組み合わせによる double-locus sequence typing（DLST 法）を用いて MRSA の遺伝子型を特定した。2005 年から 2006 年にスイス西部で分離された重複のない MRSA 分離株 1,268 株のタイピングを行った。897 株（71％）が 4 種類の優勢な遺伝子型、231 株（18％）が 55 種類の優勢ではない遺伝子型であり、140 株（11％）は単一の株であった。優勢ではない遺伝子型の分離株を保菌する患者 231 例のうち、明確な疫学的関連が認められたのは 106 例（46％）のみであり、分子サーベイランスにより特定されたクラスター数は古典的な疫学的関連が疑われたクラスター数の 2 倍であった。しかし、これらの分子的クラスターのすべてがヒトからヒトへの伝播であったことを示しているわけではない。したがって、分子タイピングは古典的な疫学データに代わり得るものではなく、補完的なものである。MRSA 遺伝子型の前向きサーベイランスは、病院環境や市中環境における新たなリスク因子や新たな流行性クローンの出現を把握するための疫学的追跡に有用であると考えられる。
