CSI: a severity index for Clostridium difficile infection at the time of admission

米国ではクロストリジウム・ディフィシル（Clostridium difficile）は院内下痢症の原因菌として高い頻度でみられる。入院時の C. difficile 感染症が重度であるか軽・中等度であるかの判定に有用なスコアリングモデルを作成するために、後向きコホート研究を実施した。2004 年 1 月から 2007 年 12 年に、255 例の患者が本研究の組み入れ基準に合致した。重度の C. difficile 感染症の定義は、結腸切除、集中治療室での管理、死亡による試験終了、または 10 日を超える入院を要する場合とした。データとして既往歴、入院時の身体診察所見、臨床検査データ、および画像データ・大腸内視鏡検査データなどを記録した。C. difficile 感染症重症度指標（CSI）のスコアの算出にあたっては、単変量解析により特定した重度の C. difficile 感染症の 4 つのリスク因子、すなわち悪性腫瘍の既往歴、入院時の白血球数 ＞ 20,000/dL、血中アルブミン値 ＜ 3.0 mg/dL、および入院時クレアチニン値がベースライン値の 1.5 倍を超えることを算入した。各リスク因子に対して1 点を割り当てた。c 統計量は 0.78 であり、CSI スコアによる重度の C. difficile 感染症の予測能は偶然（c 統計量 0.50）よりもはるかに良好であることが示された。重度の C. difficile 感染症の発現リスクは、CSI スコアの 1 点上昇ごとに 2.9 倍（95％CI 1.82 ～ 4.59）上昇した。カットオフ値が 2 の場合の CSI スコアの感度は 82％、特異度は 65％であった。CSI スコアは入院時の重度の C. difficile 感染症リスクの定量化、およびより積極的な治療が有益となる可能性がある患者の早期特定に有用であると考えられる。
