Pandemic A/H1N1/2009 influenza in a paediatric haematology and oncology unit: successful management of a sudden outbreak

ウイルス性呼吸器感染症は、癌治療を受けている小児の生命を脅かす可能性がある。小児血液腫瘍内科で発生したパンデミックインフルエンザ A（H1N1）2009 の院内アウトブレイク 6 症例について報告する。患児 3 例が肺炎を発症し、このうち 2 例に持続性心血行動態不全が認められた。感染源は、第一感染患児の親類であると考えられた。アウトブレイクの拡大は、社会活動中の患者間の交差感染によるものと考えられた。アウトブレイクの判定が数日遅延したことが、インフルエンザの拡大を助長した。感染制御策として、全入院患児に対するオセルタミビル経口投与、感染患児の隔離、厳格な個人防御策、来院者の制限などを実施した。これらの封じ込め策施行後は、新たな症例は認められなかった。アウトブレイクと判定された時点では、他の理由からすべての患児がすでに隔離されていた。結論として、インフルエンザ A（H1N1）2009 は急速に拡大し、小児癌患児の重度の感染症の原因となる可能性があるが、効率的に封じ込めることが可能である。単発症例またはアウトブレイク症例を特定することは、適切な感染制御策の迅速な実施につながる。
