Rectal culture screening for vancomycin-resistant enterococcus in chronic haemodialysis patients: false-negative rates and duration of colonisation

長期血液透析患者ではバンコマイシン耐性腸球菌（VRE）感染・保菌が多くみられる。しかし、長期血液透析患者の VRE 保菌期間や、VRE の除去を判定するための直腸培養の連続陰性の信頼性に関する情報は限られている。VRE が分離された長期血液透析患者を対象として、後向きの評価を実施した。2003 年 6 月 1 日から 2010 年 3 月 1 日に、直腸培養検体を 1 週間以上の間隔で 4 回以上採取した。VRE の連続培養の結果および患者データの解析を行った。VRE が分離された 812 例のうち長期血液透析患者は 89 例、培養結果が 3 回連続して陰性であった患者は 92 例であった。培養結果が 3 回連続して陰性であった検体の採取期間は、非長期血液透析患者 83 例では 60.7 ± 183.9 日、長期血液透析患者 9 例では 111.4 ± 155.4 日であった（P ＝ 0.011）。直腸培養結果が 4 回以上連続して陰性となる独立リスク因子は、グリコペプチドの使用（オッズ比［OR］2.155、P ＝ 0.003）および入院期間（OR 1.009、P ＝ 0.001）であった。直腸培養結果が 3 回連続して陰性であることが確認された後に再度陽性が認められた患者は、長期血液透析患者は 6 例中 2 例、非血液透析患者は 36 例中 10 例であった。結論として、3 週間隔での直腸培養ではかなりの割合の VRE 保菌患者が検出漏れとなること、および長期血液透析患者の VRE 保菌期間は長期化する傾向があることが示された。これらの結果は、VRE 保有率の持続的上昇の一因となっている可能性がある。
