大腸菌(Escherichia coli)臨床分離株に対する四級アンモニウム化合物の最小発育阻止濃度高値が抗菌薬感受性および臨床転帰に及ぼす影響

四級アンモニウム化合物は陽イオン界面活性剤であり、防腐剤および環境消毒薬として用いられる。臨床環境における四級アンモニウム化合物の効果に関するデータは限られている。2008 年 2 月から 9 月にレンヌの大学病院で、大腸菌(Escherichia coli)菌血症患者 153 例を対象とした前向きコホート研究を実施した。寒天平板希釈法により抗菌薬および四級アンモニウム化合物であるアルキルジメチルベンジル塩化アンモニウム(ADBAC)とジデシルジメチル塩化アンモニウム(DDAC)の最小発育阻止濃度(MIC)を測定した。クリスタル・バイオレット法によりバイオフィルム形成能を評価し、分離株のリファンピシン耐性発現能の測定により変異頻度を評価した。ロジスティック回帰分析から、コトリモキサゾール感性は ADBAC(≦ 16 mg/L)および DDAC(≦ 8 mg/L)の MIC 低値と関連する重要な因子の 1 つであることが示された(ADBAC のオッズ比[OR]3.72、95%信頼区間[CI]1.22 ~ 11.24、P = 0.02、DDAC の OR 3.61、95%CI 1.56 ~ 7.56、P < 0.01)。コトリモキサゾール感性であることは、アモキシシリンおよびナリジクス酸感性と強く関連していた(P < 0.01)。市中感染菌血症と医療関連菌血症のいずれであるか、菌血症の重症度、および患者の転帰は ADBAC および DDAC の MIC とは関連がなかった。今回の知見から、大腸菌臨床分離株に対する四級アンモニウム化合物の MIC 高値と抗菌薬耐性との疫学的関連が示された。
