Excess mortality following community-onset norovirus enteritis in the elderly

ノロウイルスは超過死亡をもたらす。ノロウイルス腸炎後の死亡率に関する後向き研究を実施した。症例はスウェーデン、ヨーテボリの Sahlgrenska 大学病院で 2008 年 8 月から 2009 年 6 月に PCR 法により便検体のノロウイルス genogroup II 陽性を認め、治療を受けたすべての成人入院患者とした（598 例、年齢 18 ～ 101 歳）。比較対象として、腸炎のないマッチさせた対照（1,196 例）を選択した。カルテレビューを行い、陽性検体採取日から 90 日後までの死亡、併存疾患、および入院期間を記録した。30 日および 90 日生存率を算出した。全体の 30日死亡率は 7.6％であり、18 ～ 59 歳の症例では死亡は認められなかった。60 ～ 101 歳では、基礎疾患を有する症例の 30 日死亡率は、併存疾患のない症例と比較して高かった（30 日生存率 89.5％対 94.7％、P ＜ 0.05）。80 歳を超える症例では、市中発症ノロウイルス腸炎患者（64 例）の死亡率は病院発症ノロウイルス腸炎患者（305 例）より高く（30 日生存率 81.2％対 90.2％、P ＜ 0.05）、対照（128 例）との比較でも高かった（30 日生存率 81.2％対 91.4％、P ＜ 0.05）。入院期間中央値は、病院発症ノロウイルス腸炎例 20 日（四分位範囲［IQR］ 12 ～ 29 日）、対照例 7 日（IQR 2 ～ 13 日）であった（P ＜ 0.001）。結論として、入院を要する市中発症ノロウイルス腸炎は、病院発症ノロウイルス腸炎および高齢入院患者のマッチさせた対照と比較して死亡率が高かった。
