Automated detection of nosocomial infections: evaluation of different strategies in an intensive care unit 2000-2006

本研究の目的は、種々の病院情報システム、すなわち微生物データベース、抗菌薬処方、医療管理データベース、および退院時サマリーを使用した、集中治療室（ICU）における院内感染症の自動検出のための 7 種類の方策を評価することである。本研究の対象は、2000 年から 2006 年にフランスのリヨン大学病院の ICU 入室患者 1,499 例である。データの抽出は微生物検査情報システム、病棟内の臨床情報システム、および医療管理データベースから行った。これらのデータベースを個別に、または組み合わせて使用し、種々のアルゴリズムと方策を作成した。ゴールドスタンダードである全米病院感染サーベイランスシステムに準ずる全国的な標準的サーベイランスプロトコールに従って収集した病棟データを用いて結果を比較し、各方策の性能を評価した。1,499 例の患者から 282 件の院内感染症が報告された。自動検出法による院内感染症の検出にあたって最も感度が高い方策は抗菌薬処方アルゴリズムまたは微生物アルゴリズムの併用による方法であり、その感度は 99.3％（95％信頼区間［CI］ 98.2％ ～ 100％）、特異度は 56.8％（95％CI 54.0％ ～ 59.6％）であった。院内感染症の自動検出法は、従来のモニタリング法の代替手法となることが示された。全国的な推奨法を確立するためには、より多くの ICU を対象としたさらなる研究を実施すべきである。
