Control of an outbreak of carbapenem-resistant Pseudomonas aeruginosa in a haemato-oncology unit

1,240 床の 3 次ケア施設である京都大学病院の血液腫瘍内科におけるメタロ-β-ラクタマーゼ産生多剤耐性緑膿菌（multidrug-resistant Pseudomonas aeruginosa producing metallo-β-lactamase；MBLPA）アウトブレイクを、多分野にわたる介入により制御した。本研究では、積極的監視培養により MBLPA 発生率を明らかにすることによって上記の介入効果を評価した。2004 年の感染制御策は、接触予防策の強化、MBLPA 感染のリスク因子の分析、および尿検体採取の中止などであった。しかし、2006 年に新たな MBLPA 感染が確認されたため、環境洗浄の強化、定期的な積極的監視培養、およびカルバペネム系抗菌薬の使用制限を実施した。2004 年から 2010 年に、当科で識別不能の MBLPA 株に感染した患者は 17 例であった。最終的に、定期的な積極的監視培養によって確認された感染患者は 5 例であったが、水平伝播は確認されなかった。血液腫瘍内科における MBLPA アウトブレイクは 2008 年に制御された。
