Evaluation of the sporicidal activity of different chemical disinfectants used in hospitals against Clostridium difficile

クロストリジウム・ディフィシル（Clostridium difficile）伝播の制御には表面および医療器具の汚染除去が不可欠であり、多くの製品が本菌に対する不活化効果を標榜している。32 種類の消毒薬の C. difficile 芽胞に対する効果について、欧州規格 BS EN 13704:2002 に基づく懸濁試験により、清浄条件（0.3％アルブミン）と汚染条件（3％アルブミン）のシミュレーション下で接触時間 1 分と 60 分で評価した。欧州規格で定められた接触時間 60 分に、接触時間 1 分での評価を追加した理由は、本研究モデルが想定している実際の曝露条件のより現実的なシミュレーションを行うためである。製造者の推奨濃度の最低値で試験を行った。16 製品では、清浄条件および汚染条件での 60 分後の生残芽胞数減少 ＞ 10³ （欧州規格の合格基準）に到達した。しかし、汚染条件で 1 分以内に生残芽胞数減少 ＞ 10³ に到達したのは 8 製品のみであった。3 製品では、いずれの試験条件でも C. difficile 芽胞生残数は 10³ 分の 1 に減少しなかった。本研究から、殺芽胞効果を標榜する消毒薬それ自体は環境中の C. difficile を制御するための万能の解決策ではないが、慎重に選択された環境消毒薬は、各種洗浄戦略を含む多くの制御策の一部を担うことができると考えられる。

監訳者コメント：

英国では二酸化塩素が環境消毒に使用されているため，本評価でも多くの検証が行われている。一方米国では、6,000 ppmの次亜塩素酸ナトリウムや蒸気化過酸化水素が用いられており、国により状況は様々である。
